headphone "echo" and sounds

Works nice with XLR and 1/4" output.
The headphone output makes a kind of echo and annoying sounds (not a hum or static). Obviously the headphone amp inside is defective.

I do not use the headphone output of the item but another one behind the XLR. But this is a stopgap.
